Radeon Driver crashing on OS4 Final

Recently, I have witnessed several occasions when the Radeon driver crashes on boot. Just before WorkBench should appear, I get a visit from Grim Reaper and am forced to reboot the machine. Today, I had to reboot three times in a row before the machine booted properly. Here is the crash log:

Any ideas what could cause this? I have checked the Radeon tool types between the final and Update 4 and cannot see any alarming differences:

I'd guess that it's nothing to do with the Radeon driver, since it only appears a long way down in the stack trace.

What DOES appear (recently) in the stack trace is icon.library calling DOS for something or other.

Have you got any funny icons and/or icon hacks running?
Why not remove them and see if it is any better?

No icon hacks, only the png.iconmodule in 'LIBS:IconModules/'. I use png icons from the PowerIcons archive.

The 640x480 frequency does seem to be wrong (it should be max 80) but the 1280x1024 freq is right for the Acer TFT I use.

@pvanni
The 'softsprite' and 'interrupt' tooltypes are at their default setting - as the installer set them. What do these tooltypes do? I have understood that you need to have interrupt=yes if you want the Warp3D games to work well.

Today the system has not crashed at all - no GRs during boot. There seem to be no simple reason for this behaviour

I seem to remember that powericons should be removed from your startup sequence
or user-startup. OS4 no longer needs the hack but be sure to install the svg datatype.
Give that a go.
