Notifications page not working (blank)

If I go to User Menu > Notifications, it takes me to a blank page:

When I receive email notifications about new posts in a thread, it also links to that non-working page:

So currently the only way to unsubscribe from a thread is to edit a post you've made to that thread, and submit it after unticking "Notify me of new posts in this thread" (which leaves a pointless note at the bottom of the post saying "Edited by YOURNAME on YYYY/MM/DD HH:MM:SS").
